INTRODUCTION

These Terms and Conditions govern the use of the Midland Chiropractic Care website and the provision of services by Midland Chiro Clinic Pty Ltd, trading as Midland Chiropractic Care.

By accessing this website, booking an appointment, or attending the clinic for consultation or treatment, you agree to be bound by these Terms and Conditions.

Midland Chiropractic Care may update these Terms and Conditions from time to time. The most current version will be available on the clinic’s website.

WEBSITE INFORMATION

The information provided on this website is intended for general informational and educational purposes only.

Content on this website does not constitute medical advice, diagnosis or treatment and should not be relied upon as a substitute for consultation with a qualified healthcare practitioner.

Health conditions vary significantly between individuals, and information on this website may not be appropriate for every person or circumstance.

PRACTITIONER-PATIENT RELATIONSHIP

Use of this website does not establish a practitioner–patient relationship between the user and Midland Chiropractic Care.

A practitioner–patient relationship is established only when a patient attends the clinic for consultation and appropriate clinical assessment and consent procedures have been completed.

APPOINTMENT BOOKINGS

Appointments may be made through the clinic directly (via any reasonable means) or through online booking platforms such as HealthEngine.

Patients are responsible for providing accurate and complete information when booking appointments.

Appointment times are scheduled to allow appropriate consultation and treatment. While Midland Chiropractic Care makes every effort to maintain appointment schedules, unforeseen circumstances may occasionally result in delays.

APPOINTMENT CANCELLATIONS AND MISSED APPOINTMENTS

Patients are requested to provide reasonable notice if they are unable to attend a scheduled appointment.

Failure to attend an appointment or cancelling with insufficient notice may result in a cancellation or missed appointment fee. The purpose of such fees is to compensate for appointment time that cannot be offered to other patients.

Midland Chiropractic Care reserves the right to apply such fees where appropriate.

LATE ARRIVALS

Patients who arrive late for an appointment may have their consultation time reduced in order to avoid delays for other patients.

Where insufficient time remains to conduct an appropriate consultation or treatment, the appointment may need to be rescheduled and fees may apply in accordance with this policy.

FEES AND PAYMENTS

Consultation fees are payable at the time of the appointment unless other arrangements have been made.

Fees for services may change from time to time. Current consultation fees are available from the clinic upon request.

Patients who are eligible for rebates through health funds or other compensation schemes are responsible for ensuring that their claims are submitted appropriately. Any gap payments or outstanding balances remain the responsibility of the patient unless otherwise confirmed by the relevant organisation.

INSURANCE, COMPENSATION, AND THIRD PARTY CLAIMS

Where treatment relates to insurance or compensation claims, including WorkCover matters, Midland Chiropractic Care may require confirmation of claim approval before billing a third party.

Unless otherwise confirmed in writing, patients remain responsible for payment of consultation fees.

MEDICAL RECORDS

Clinical records created during the course of treatment are the property of Midland Chiropractic Care and are maintained in accordance with legal and professional obligations.

Patients may request access to their personal health records in accordance with the clinic’s Privacy Policy. In some circumstances, a reasonable administrative fee may apply for the preparation and provision of records.

Requests for medical reports, legal documentation, or detailed summaries may also incur administrative or professional fees.

PRIVACY

Midland Chiropractic Care is committed to protecting patient privacy and handling personal information in accordance with the Privacy Act 1988 and the Australian Privacy Principles.

Details regarding the collection, use and storage of personal information are outlined in the Midland Chiropractic Care Privacy Policy, which is available on the clinic’s website.

Midland Chiropractic Care is committed to providing a safe and respectful environment for patients, practitioners and staff.

The clinic reserves the right to refuse treatment or terminate services where a patient engages in behaviour that is abusive, threatening, inappropriate or unsafe.
